Insurance Panels

We are In-Network with many major insurance companies. We’ll contact your insurer and verify terms so you know what to expect. We do our best to make the insurance side of our services stress-free for you.

Amplified Life is an In-Network provider with the following insurance companies and Employee Assistance Plans (EAPs):

  • ASR
  • Beacon
  • Blue Care Network (BCN)
  • Blue Cross Blue Shield of Michigan (BCBSM)
  • Magellan
  • Molina
  • Optum (formerly United Behavioral Health)
  • Tricare East

If your insurance provider isn't listed here, give us a call (616) 499-4711. We're adding new panels as often as possible!

We understand that sometimes clients do not have insurance coverage or prefer to simply leave the insurance company out of it by paying cash for services received. We accept checks and credit cards as well.

*Our standard rate for 55-60 minute sessions is $140 (this may vary based upon your particular insurance plan).

Counseling with Licensed Behavioral Health Therapists:

Standard insurance reimbursement rates and private pay options available for counseling services.

Coaching with Qualified Life Coaches

Private pay rates range from $50-$150 per hour based on nature of coaching requested.

What To Ask Your Insurance Company

Some insurance plans will cover “Out of-Network” providers. If you plan on using your insurance benefits for behavioral or mental health services, simply contact your insurance company to verify your benefits and coverage and ask the following questions:

  • Do I have outpatient mental health benefits?
  • Do I have a deductible to meet for in or out of network benefits?
    • If yes: How much is my total deductible?
    • If yes: How much of my deductible has been met?
  • What is my copay amount per session?
  • Can I be seen by a LLPC, LPC, LLMFT, LMFT, or PhD?
  • Does my insurance allow supervision by a PhD or LMSW?
  • Is my therapist in or out of network?
  • If my therapist is out of network, what will my insurance cover for out of network services?
  • How many visits am I allowed per year?
    • If there is a limit on visits, how many have I used already?
  • What is my policy plan year (e.g., January to January or July to July)?
  • Do I need a referral or authorization in order to be seen for therapy or testing?
  • Will my insurance cover individual and group therapy on the same day?
